چکیده
Abstract Background and Aims Previous studies in chronic kidney disease (CKD) showed that vascular dysfunction different circulatory beds is progressively deteriorating with CKD severity. This study evaluated muscle oxygenation microvascular reactivity at rest, during an occlusion-reperfusion maneuver, exercise patients pre-dialysis versus controls, as well between stages Method Continuous measurement of [tissue saturation index (TSI%)] via near-infrared-spectroscopy occlusion-reperfusion, a 3-min handgrip exercise(at 35% maximal-voluntary-contraction). Aortic pulse wave velocity (PWV) carotid intima-media thickness (cIMT) were also recorded. Results Resting did not differ among the groups (controls: 64.3±2.9 stage-2: 63.8±4.2stage-3a: 64.1±4.1 stage-3b: 62.3±3.3 stage-4: 62.7±4.3%; p = 0.6). During occlusion, no significant differences detected TSIocl magnitude- occlusion-slope. However, reperfusion, theTSImax hyperemic response significantly lower more advanced 11.2±3.7 8.3±4.6 stage-3: 7.8±5.5 7.3±4.4 7.2±3.3; 0.04). exercise, (TSIaverage-decline) was marginally than but stages. Conclusion Although observed rest reperfusion impaired deteriorated ability microvasculature to respond stimuli may be crucial component adverse profile contribute intolerance

Near-infrared spectroscopy for monitoring muscle oxygenation.
Near-infrared spectroscopy (NIRS) is a non-invasive method for monitoring oxygen availability and utilization by the tissues. In intact skeletal muscle, NIRS allows semi-quantitative measurements of haemoglobin plus myoglobin oxygenation (tissue O2 stores) and the haemoglobin volume. Specialized algorithms allow assessment of the oxidation-reduction (redox) state of the copper moiety (CuA) of m...
متن کاملNear-infrared spectroscopy for assessing tissue oxygenation and microvascular reactivity in critically ill patients: a prospective observational study
BACKGROUND Impaired microcirculatory perfusion and tissue oxygenation during critical illness are associated with adverse outcome. The aim of this study was to detect alterations in tissue oxygenation or microvascular reactivity and their ability to predict outcome in critically ill patients using thenar near-infrared spectroscopy (NIRS) with a vascular occlusion test (VOT). METHODS Prospecti...
متن کاملNear infrared spectroscopy for evaluation of skeletal muscle tissue oxygenation in different types of shock
Clinical examination is non-invasive, but has well-recognized limitations in detecting compensated and uncompensated low flow states and their severity. This paper describes the principles of near infra-red spectroscopy (NIRS) and the basis for its proposed use, in hypovolaemic, cardiogenic and septic shock, for assessing global and regional tissue oxygenation. The vascular occlusion test is ex...
متن کاملSerum Irisin level in patients undergoing different Stages of Chronic Kidney Disease
Background: Irisin is a myokine that regulates energy metabolism by inducing browning of adipose tissue. The aim of this study was to evaluate the relationship between irisin level and biochemical parameters of chronic kidney disease (CKD) patients in stage 2 and stage 4. Methods: The research was a cross-sectional study; the study population included patients with CKD who were over 18 years o...
متن کاملCerebrovascular reactivity measured by near-infrared spectroscopy.
BACKGROUND AND PURPOSE The pressure reactivity index (PRx) describes cerebral vessel reactivity by correlation of slow waves of intracranial pressure (ICP) and arterial blood pressure. In theory, slow changes in the relative total hemoglobin (rTHb) measured by near-infrared spectroscopy are caused by the same blood volume changes that cause slow waves of ICP. Our objective was to develop a new ...
